Involvement in Client Care Team

• Nursing & Homemakers will assign a primary care provider to each client., who is then accountable for ensuring the client's treatment goals are met and for communication with the client.
• NHI will ensure that there is a plan for providing continuity and consistency of service to clients.
  
• NHI will have an open oral and or written communication vehicle with clients, physician and other team members.
